Friendships .. how do u know when to take it to the next level?

I recently realized that I'm really good at surface talk .. meaning just meeting people, asking for their names, jobs, hobbies, small talk, and anything that's not too personal. When do you know to take it to the next level? It's easy to ask for a phone number and say "we should get together some time", but it's so difficult for me to follow thru. And then it's embarrassing when you run into them again and their name/number has been collecting dust on your phone.
